What Happens in Dr Tolbert's Program?

We Get You Movin' and Feelin' Better!

1. DETERMINE MEDICAL AND FUNCTIONAL (REHABILITATION) DIAGNOSES

Your condition, needs, and expectations, are thoroughly assessed by the M.D, and team. It is important to first rule out any serious medical illnesses before a treatment plan can be developed.

2.TAILORING A PLAN WITH THE RIGHT TYPE OF EXERCISES TO EFFECTIVELY OVERCOME FITNESS OBSTACLES.

Exercise, like a medication, should be a thoughtful prescription. For example, the exercise program prescribed for a 45 year-old patient with back pain and Fibromyalgia, is not the same prescription which is prescribed to a 30 year old patient, one week after an epidural has been preformed.

Therapy is prescribed and performed onsite, which allows evaluations on how you are responding to therapy. This allows any immediate necessary changes in the therapy prescription, avoiding potential injury, or wasted therapy sessions.

As a rehabilitation physician, I might prescribe medication, stretching, and massage for short-term pain relief, as well as strengthening exercises to prevent future pain. In order to assess medications or my pain procedures i.e joint injections, epidurals, etc, I evaluate how patients respond to therapy. If surgery is a necessity, I work with patients and their surgeons before and after surgery. By directing my treatment team and collaborating with other health care professionals, I am able to specially design a treatment program tailored to the patient and expedite healing.

3.UNDERSTANDING AND IDENTIFYING YOUR GOALS

Common goals are strengthening an injured muscle; finding relief from chronic pain; reducing or not increasing pain medications; avoiding surgery or walking up the stairs without being winded. As a rehabilitation physician I work with patients to determine realistic short- and long-term goals. Along the way, we will help you to find relief from pain, achieve successes in rehabilitation or exercise programs, overcome your setbacks, and reassess your goals if necessary.

4. CREATING INDEPENDENCE

The final goal is helping patients become independent with the prescribed exercise program. The incredible health benefits of exercise are well known . In this final stage, I ensure that patients can continue my exercise prescription safely at home, their gym, or wherever they will be exercising. Patients' readiness for discharge is assessed by having them demonstrate their program safely. I re-assess the patient in one month and thereafter as needed. Regular Check-ups are recommended in order to prevent re-injury and maintain progress.
